I agree to have my presence mentioned in third-party post-conference publications.
CPM is frequently asked by third-party publication to write articles to follow-up on conference proceedings. The articles will appear in national and international publications. We request your permission to reference your presentation in post-conference publications, giving full attribute to your accomplishments and participation in the event.
Release Authorization
By signing this document, you consent to the use of your name, biography (if supplied), and photograph(s) in CPM literature and elsewhere, for the purpose of publicizing the organization. By signing this agreement, you take sole responsibility and accept the sole risk of any information that you disclose - publicly or otherwise - through your participation with CPM. You release CPM, and its affiliates, directors, officers, employers, representatives, agents and agencies from any and all liability, losses, damages, claims, and expenses of any nature that you may suffer or incur in connection with participation in the conference affiliated with CPM.
